The membership is valid for one year from its day of commencement.
You can view or check the balance of your cashback in the following ways:
1. Gold & Silver Club Members can call our customer care team on 022-6170000 and the Platinum Club Members can call their dedicated concierge on 022-6170001 to enquire about their balance and the respective teams will help with same
2. Sign in with your registered mobile number onto our website and check the wallet icon on the top right corner
3. Walk into a Suburban Diagnostics centre and meet our reception staff
Your membership discount coupons cannot be clubbed together. You can use multiple coupons on multiple registrations. If you choose to do multiple registrations for yourselves or your loved ones on a single day, you can use multiple coupons on those multiple registrations.
We do charge a nominal fee of Rs. 150 for home visits on tests below Rs. 2,000. However, if your total bill amount is above Rs. 2,000, no home visit charges will be applicable
Yes. You can use discount coupons while you are redeeming your wallet balance.
Currently, your membership, its discounts and its benefits are only valid in the centres mentioned here.
Yes, home visit charges can be redeemed from your wallet.
No. At any given point of time only one of the multiple offers running at a given time can be availed.
No. The wallet balance stays in your wallet even after your membership expires and can be consumed by you.
Terms & Conditions
- The Membership & it's benefits are valid only in Mumbai & MMR at the mentioned centers and during home visits.
- The membership is valid for one year from its commencement date.
- The health check-up in your membership wallet can only be used by the primary member and not transferable.
- ProSelf Health Check-up booked under this scheme is non refundable.
- Any ProSelf package's composition cannot be altered.
- The discount coupons are not valid on health check-up packages and COVID-19-related services.
- The coupons are valid only until your ProSelf Club Membership is valid. Please save the coupon codes. It is mandatory to provide the coupon codes in order to avail this offer. The staff at Suburban Diagnostics does not have access to the coupon codes provided to you.
- All complimentary consultations will be telephonic in nature.
- Priority booking for body imaging services can be availed with a 24 working hrs. prior intimation and priority booking for home sample collection can be availed at 6 working hrs. prior intimation.
- Annual comprehensive health report will be prepared only for the primary member.
- Membership discount coupons will be sent to the primary member only.
- No two offers can be availed together (Discount coupons cannot be clubbed).
- Home visit charges as applicable.
- The offer is open only to Indian citizens.
- The offer can be revoked/refused if an individual is found involved in acts of fraud or cheating, without any notice.
- The offers may be withdrawn without prior intimation. Suburban Diagnostics shall not be responsible for any liability arising from such situations.
- Suburban Diagnostics reserves the right to amend these terms and conditions and is visible here at any time without any prior notice. Modifications of these terms will be effective from the time they are updated.
- Any dispute arising will fall under the jurisdiction of courts in Mumbai only.
